Fax: Download a printable form in either Word or PDF format from the links below. Print either version, fill it out and fax per the instructions on the form. Send your logo requiring digitizing (setting up your logo in an electronic file that embroidery machines can use) via email to to logos@embroideryauthority.com.

2. We prepare your complete quote and send to you by e-mail or fax, usually the next business day. It will list:
- the price of all items requested
- cost of digitizing or setting up your logo (a one-time fee, if necessary)
- sales tax (within Florida only)
- shipping (if you give us a shipping address)
- our ability to meet your
in-hands date (if a delivery date is specified).
3. To accept the quote, simply reply to the e-mail and let us know that
you want to place the order. Indicate any changes (items, quantities,
colors, sizes, thread colors, etc.) that need to be made. Ensure that we
have an accurate UPS shipping address. We'll prepare an invoice which we
will e-mail to you. Included will be a link to a secure website where you
can pay by credit card. We require a minimum of 50% payment on your order
as a deposit to start the order process, or you may pay the invoice all at once
if you so desire.
4. We will digitize your logo, sew it out onto fabric, take a
digital picture of it, and e-mail it to you for approval within 2 business days
of our verification of your payment. If you are ordering heat transfers,
we will setup your logo and e-mail you a reduced size printout for approval.
Edits to the logo are free until it's to your satisfaction, as long as it's not
different from the original artwork.
5. Once we have
your approval of the logo, we ask that you pay the remainder of your invoice
as quickly as possible to avoid any delay in shipping your order since we
require payment in full before shipment. Once we have verified full
payment, we'll embroider or heat transfer your order and ship it out to you within two weeks.
